Is there only a little madecassoside if it’s near the end of the ingredient list?

Generally, yes, but the order alone doesn’t tell you the percentage. In Korea, Annex 4 of the Enforcement Rule of the Cosmetics Act (amended February 7, 2025) requires ingredients to be listed from the largest amount down, while ingredients used at 1% or less, fragrances and colorants can be listed in any order.

So among the ingredients bunched toward the end of the list, the order may not reflect amount. A practical way to read it is only to tell apart whether a centella ingredient shows up within a few slots after basics like water and glycerin, or sits near the back by the fragrance listing. Other countries have their own labeling rules, so if you want the exact amount, asking the manufacturer’s customer service is the surest route.

How is centella asiatica leaf water different from centella extract?

In the Korea Cosmetic Association ingredient dictionary, Centella Asiatica Extract is defined as an ingredient extracted from the whole centella plant, and Centella Asiatica Leaf Water as a water solution obtained by steam-distilling centella leaves. Madecassoside is registered separately, not as a plant name but as a single organic compound with a defined structure.

Different names mean different raw-material forms, not that one is better. When you compare two products, compare the list positions of ingredients with the same name, and just keep in mind that a product with Centella Asiatica Leaf Water at the very top may be using it as the base in place of water.

Where does cica cream go in your routine?

For a cream, it usually goes on after washing and toner, as the last step of your routine. The Korean Dermatological Association explains that toner supplies moisture first after washing, and cream temporarily adds a protective layer to the skin to shield it from outside irritation.

With a spot product, dab it only on the breakout first and give it a moment, then spread cream around it so the two don’t pill. No study has compared the order itself, so if you change the order, note it in your log with the date.

When do you add it and drop it for seasonal or mask breakouts?

Add it only for a stretch with a clear trigger, and when that stretch ends, look at 2 to 4 weeks of notes to decide whether to drop it. Human studies involving centella also watched over several weeks. In a 2021 trial in which 30 textile dye workers in Indonesia with dry hands applied a centella cream or a ceramide cream twice a day, both creams improved hydration and pH measures on the backs of the hands and palms after 4 weeks, with no difference between them.

If it’s a winter of long mask days or a seasonal change with big day-night temperature swings, write down the start date and the trigger together. A 2024 trial in which 64 people with mild to moderate rosacea added a centella mask (3 times a week) to an oral medicine and a moisturizing cream evaluated them twice over 6 weeks, at week 3 and week 6; everyone was also taking the oral medicine, so the mask’s own share can’t be known. At home too, setting dates like week 3 and week 6 and comparing photos and notes makes the add-or-drop call easier.

What mixes did the cica products in studies use?

Most were finished products mixing several ingredients rather than centella alone. In a 2021 placebo-controlled trial in which 51 people with darker skin tones (Fitzpatrick types IV and V) and post-acne hyperpigmentation used 3 drops twice a day for 8 weeks, the serum was also a mix of galactomyces ferment filtrate, panthenol and centella.

Madecassoside is the same story. In a 2019 study where 20 people applied a cream to half the face for 7 days right after a procedure, the cream contained 5% panthenol along with madecassoside, copper, zinc and manganese. So when you choose a cica cream, add a line to your log about what else is in it besides the centella ingredient.

Log only one trigger at a time so you can compare what adding and dropping it did. Don’t bring in other new products during the same period.

When should you get a dermatologist involved?

• If red spots or swelling appear where you applied cica cream, stop using it and see a specialist.
• If itching and redness continue 1 to 2 weeks after you drop the product, ask a dermatologist about finding the ingredient responsible (patch testing). The Korean Dermatological Association lists plants like the lacquer tree along with cosmetic fragrances and preservatives as causes of allergy.
• If redness keeps coming back for weeks even as you add and drop cica cream, see a dermatologist rather than trying to work out whether the cream is to blame.
• If deep bumps that hurt when pressed or marks increase, consider seeing a doctor before switching creams.
