| Question | Detail | Basis |
|---|---|---|
| How common is it? | Of 696 acne patients in five US cities, about 50% also had spots on the chest or back, and about a quarter of those with both face and body acne did not bring up the body spots on their own | 2007 PMID 17668525 · B |
| People who work out | 33.2% of gym users in Saudi Arabia had exercise-related skin problems, and friction-related acne was seen in 25% | 2025 PMID 41234265 · B |
| Summer | Oil output rises 10% for every 1°C (1.8°F) rise in body temperature, so acne that had calmed down can flare in summer | Korea Disease Control and Prevention Agency (KDCA) · Korean agency |
| Irritation to avoid | The hospital advises avoiding mechanical irritation such as pressure and friction, and heavy sweating | Seoul National University Hospital · Korean hospital |
| How long to log | Even a study of 43 people with back and chest acne who used a lotion tracked scores and skin condition for 56 days (8 weeks), with no control group | 2024 PMID 39044362 · B |
What order should you shower in after a workout?
Take off wet clothes first, wash your hair next, and use body wash last; that order also makes your log easier to keep. Seoul National University Hospital, a university hospital in Korea, describes heavy sweating and friction as things that make acne worse, and the time you spend moving around in a sweat-soaked workout shirt is when both overlap.
If you ride the train home from the gym still in your workout gear, or you pull regular clothes on over a sweaty shirt after gym class, that stretch is the first box in your log. Shampoo and conditioner suds run down your neck onto your back, so finish your hair first, rinse your back once, and then use the body wash. If you use body wash first, hair product residue can sit on your back until the very end.

How do you wash your back when you can’t reach it?
Put lather only on the shoulders, shoulder blades, and sides you can reach, and let the suds that run down plus the shower water rinse the area around your spine. Scrubbing hard-to-reach spots with a long brush or a rough exfoliating mitt does not fit with the Korean Dermatological Association listing friction and mechanical irritation as things that make acne worse.
Work the body wash into a lather in your palms first, lay it on your skin, glide over it a few times, and rinse right away. It is a rinse-off product, and the sources behind this article do not show that leaving it on longer does any better. For a leave-on back acne body lotion or body mist, a spray format, or asking someone at home to lay it on with a flat palm, is gentler than a scrubbing tool.

Why do I get back acne in the summer?
In summer, oil, sweat, and friction from clothes and bag straps all go up together, stacking the conditions for back breakouts. The Korea Disease Control and Prevention Agency (KDCA), Korea’s national public health agency, explains that oil output rises 10% for every 1°C rise in body temperature, so acne that had calmed down can get worse in summer, and Seoul National University Hospital names heavy sweating, pressure, and friction as aggravating factors.
So in your log, write down the day’s conditions rather than the season. Note how long you sweated, how long you wore a backpack or bag strap, and how long you wore a clingy workout top, and even within one summer the flare days will start to share something. Since friction-related acne showed up in 25% of gym users in the survey, mark where seat backs or equipment touched you on workout days. The KDCA notes that acne also appears on the neck, chest, back, and shoulders, so track chest breakouts in the same log, and if small, even, itchy bumps pop up after you sweat, check the visit guide below first.
How do you take back photos over four weeks?
Every week, on the same day, in the same light and the same spot, take one photo of your whole back and one of the area with the most breakouts, and collect four weeks of them. In a 2024 report, the UK Acne Working Group, a panel of UK acne specialists, suggested looking not only at the count when judging acne severity but also at location, lesion size, and the effect on how you feel.
If it is hard to take the photo yourself, set your phone timer to 10 seconds and stand it on a shelf at the same height each time. Next to each photo, write the number of new spots, the size of the biggest one (grain of rice or pea), and whether it hurts. A 2024 study of 43 people with back and chest acne who used a lotion followed changes for 56 days, which shows that body skin takes time to judge; if you switch body washes, change only one condition for at least four weeks.
What should I do about a hard pimple on my back?
Do not squeeze a lump that feels hard; write down its size and pain, and if it grows or hurts, have a dermatologist check it. Seoul National University Hospital explains that forcing it can burst the follicle wall weakened by inflammation, spreading the inflammation and making scars more likely.
Body washes and lotions are products you rinse or apply on the surface, so they do not reach a lump deep in the skin directly. In the shower, just lay lather over that spot without pressing it with your fingertips, and mark it separately in your photo log so you can tell a doctor when it started.
Four-week back photo log
| Week | The one thing you change | Photo day | What to write down |
|---|---|---|---|
| Week 1 | Only the shower order (wet clothes → hair → body) | Sunday night, same light | New spots, number of workout days |
| Week 2 | Keep week 1 + change clothes after workouts | Sunday night | Hours with a bag or backpack on, size of the biggest spot |
| Week 3 | Keep week 2 (same body wash) | Sunday night | Any painful lumps, count on the chest |
| Week 4 | Keep week 3 | Sunday night | Compare side by side with the week 1 photo |
| Week 5 on | If nothing has changed, switch only the body wash | Same day each week | Date you switched, stinging or dryness |
Flag weeks with unusually heavy sweating, like a tournament weekend or a summer trip, next to the table. If you change two or more things in one week, it is hard to tell which one made the difference.
When does back acne call for a dermatologist?
- If you get a hard lump that hurts even when you are not pressing it, or it grows, see a dermatologist.
- If the count keeps rising or spreads to your chest and shoulders after four weeks of logging, talk with a dermatologist.
- If small, even, noticeably itchy bumps appear all at once after sweating, it may be something other than acne, so get it checked by a doctor.
- If spots you squeezed or scratched start leaving dents or dark marks, see a dermatologist.
What this guide draws on
- A 2007 study of 696 acne patients in five US cities (PMID 17668525) found that about 50% also had spots on the chest or back.
- A 2025 survey of gym users in Saudi Arabia (PMID 41234265) found that 33.2% had exercise-related skin problems, with friction-related acne in 25%.
- A 2024 study that followed 43 people with back and chest acne for 56 days (PMID 39044362) had no control group, so this article uses it only as a reference for how long to log.
Frequently asked questions
How do you wash your back when you can't reach it?
When washing, put lather only on the shoulders and sides you can reach, and let the suds that run down plus the shower water rinse the middle of your back. That is enough. Seoul National University Hospital lists mechanical irritation such as friction as something that makes acne worse, so scrubbing with a long brush to reach every spot is not advised. For leave-on lotions or mists, use a spray, or ask someone at home to lay the product on with a flat palm.
Why do I get back acne in the summer?
In summer, oil, sweat, and friction all go up at once. Korea's national health agency (KDCA) explains that oil output rises 10% for every 1°C rise in body temperature, which can make acne worse in summer, and Seoul National University Hospital names heavy sweating, pressure, and friction as aggravating factors. Log your sweaty hours and the time a bag strap or seat back pressed on you, and the days that flared will start to show a pattern.
What should I do about a hard pimple on my back?
First, do not press or squeeze it; write down its size, whether it hurts, and how many there are. Seoul National University Hospital explains that forcing a pimple can burst the weakened follicle wall, spreading inflammation and making scars more likely. If it hurts without being touched, or a hard lump grows or multiplies, see a dermatologist.
