Should you scrub back acne, or leave it alone?

While back acne is flaring, it is better to take a break from scrubbing hard with an exfoliating towel. In a 2018 study, on the side rubbed with a nylon towel twice a day for 2 weeks, water loss rose and natural moisturizing factor dropped, compared with the side washed with soap alone.

Back skin also tends to react more easily. In a 2019 body-site comparison in healthy adults, the same cleansing ingredient caused more water loss (80.2 vs. 48.0 g/m²h) and more redness on the upper back than on the arm. In Korea, many people scrub their backs with a rough exfoliating towel at the bathhouse every weekend. Should you wash your hair before your body in the shower?

Why does back acne show up in winter too?

Many people get worse in summer, but back acne can appear in winter as well. KDCA explains that in winter, slower metabolism can thicken the outer skin layer, and if that clogs pores, acne can get worse.

In summer, oil output increases by 10% for every 1°C rise in body temperature. In winter, it is common to wear several fleece-lined layers, sweat on the commute, and not change until evening. When is back acne worth a dermatology visit?

  • When deep lumps that hurt when pressed form on your back or chest. The UK Acne Working Group (2024) covers acne on the trunk as its own topic, so there are aspects to look at separately from the face; consider seeing a dermatologist.
  • If red rings form around individual hairs or itching stands out, it may be something other than acne, so have a dermatologist check it.

  • If scrubbing leaves stinging, red patches over a wide area for several days, stop rubbing and get medical advice.
